About Us

Unite Hawaiʻi is a coordinated care network of health and social service providers. Partners in the network are connected through Unite Us‘ shared technology platform, which enables them to send and receive electronic referrals, address people’s social needs, and improve health across communities. Unite Hawaiʻi is sponsored by Kaiser Permanente and AlohaCare. The network is supported by a Hawaii-based Unite Us team focused on community engagement, network health and optimization, and customer success.

Joining the network is at no cost for community-based organizations, community health centers, and social service agencies.

Community Roots

Unite Hawai’i is live statewide and continues to grow each day! Join us as we onboard nonprofit, social service, and healthcare partners as the network continues to meet the needs of the community. Hale Na`au Pono has partnered with Unite Us to join the Unite Hawaii network as a champion.

We're so happy to be a part of Unite Hawai'i because not one organization can help with all of the interconnected aspects of someone's life. One part of their life can have big effects on everything else, and being able to work with other nonprofits and refer clients to each other will allow us to each help people with every one of those aspects.

Unite Us will benefit our community by giving us a platform to address the barriers and challenges our students and communities face. By connecting students/clients to resources and programs, we are able to help them get the support they need. Likewise, we are able to receive referrals for those who could benefit from our services. Through the Unite Us network, we are able to expand our outreach. A partnership with Unite Us brings collaboration across the state and helps us to connect with other agencies and organizations, ultimately providing a venue for us to extend our mission of helping to uplift our community.
